Our Commitment to Your Privacy
Nusbaum Medical Centers of New Jersey is committed to protecting the privacy of your protected health information (PHI). We are required by law to maintain the privacy of your PHI, to provide you with this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.
How We May Use and Disclose Your Health Information
Treatment. We use your health information to provide and coordinate your medical care among physicians, staff and other providers involved in your treatment.
Payment. We use and disclose your health information to bill and obtain payment from you, insurance companies or third parties.
Health Care Operations. We use your health information for quality assessment, staff review, scheduling, business operations and other administrative purposes.
Appointment Reminders & Communications. We may contact you with appointment reminders, treatment alternatives or health-related benefits and services, consistent with your communication preferences.
Your Health Information Rights
You have the right to: request restrictions on certain uses and disclosures; request confidential communications; inspect and obtain a copy of your health information; request an amendment to your records; receive an accounting of certain disclosures; and obtain a paper copy of this notice. To exercise these rights, contact our Privacy Officer using the information below.
Uses and Disclosures Requiring Your Authorization
Most uses and disclosures of psychotherapy notes, uses and disclosures for marketing purposes, and disclosures that constitute a sale of PHI require your written authorization. You may revoke an authorization in writing at any time.
Our Responsibilities
We are required to maintain the privacy of your PHI, notify you in the event of a breach of unsecured PHI, and abide by the terms of this notice. We reserve the right to change this notice and to make the revised notice effective for health information we already have, as well as any information we receive in the future.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer at 197 Ridgedale Ave, Suite 160, Cedar Knolls, NJ 07927 or (973) 998-9833, or with the U.S. Department of Health and Human Services. You will not be penalized or retaliated against for filing a complaint.
